Peripheral artery disease happens when the arteries that supply blood to your limbs become narrowed or blocked, often by fatty deposits called plaque. If untreated, PAD can progress to CLI, a serious condition marked by severe pain, non-healing wounds, or even gangrene.
Arterial revascularization refers to medical procedures that reopen blocked or narrowed arteries. Revascularization significantly reduces symptoms, helps heal vascular wounds, and decreases the risk of amputation.
There are two main types of arterial revascularization:

Endovascular Revascularization
Endovascular revascularization is a minimally invasive treatment that involves threading a thin tube (catheter) through a small puncture in your skin into the affected artery. Endovascular procedures typically mean shorter hospital stays and quicker recovery times compared to traditional surgery. Common endovascular treatments include:
Angioplasty
A tiny balloon is guided to the site of the blockage through the catheter. Once positioned, the balloon is inflated, compressing plaque against the artery walls and widening the artery to improve blood flow. After deflation, the balloon is removed.
Stenting
Often done alongside angioplasty, a stent (a small mesh tube) is placed into the artery to keep it open after the balloon has widened it. The stent provides ongoing structural support, preventing the artery from narrowing again.
Atherectomy
This procedure uses specialized tools guided through the catheter to physically remove plaque from the artery walls. Atherectomy is particularly useful when arteries have severe plaque buildup that isn’t easily compressed by a balloon. Atherectomy will typically be done alongside angioplasty.
Surgical Revascularization
Surgical revascularization procedures are more invasive surgeries that are done through major incisions and dissection techniques, often requiring longer hospital stay and recovery time compared to endovascular procedures.
Surgical Bypass
Your surgeon creates a detour around the blocked artery using either your own vein or a synthetic tube. This allows blood to bypass the blockage and flow smoothly to your limbs. Surgical bypass is especially effective when blockages are extensive or complex.
Endarterectomy
During this procedure, the surgeon opens the affected artery and directly removes the plaque blocking blood flow. Endarterectomy is particularly beneficial when there is a localized, accessible blockage that can be clearly removed to restore blood flow.
